**Privacy Agreement**
**Agreement Date:**
Effective upon signing.
**Agreement between:**
Arizona Developmental Disability Planning Council, hereinafter referred to as "ADDPC," and [Participant's/Participant's Guardian's Full Name], hereinafter referred to as the "Participant."
**Background:**
Through The Arc of Tempe, ADDPC offers a Self-Advocacy Leadership Program that includes video courses and workbook materials (hereinafter referred to as "Program Materials"). The Program Materials are designed to facilitate self-advocacy skills development for individuals with disabilities.
**Ownership of Program Materials:**
The Program Materials, including but not limited to video courses, workbooks, and related content, are and shall remain the exclusive property of ADDPC and The Arc National. The Participant acknowledges that they have no ownership rights, title, or interest in the Program Materials, except for the limited license granted herein.
**Limited License:**
ADDPC and The Arc National grant the Participant a limited, non-exclusive, non-transferable license to use the Program Materials for personal, non-commercial purposes only. This license is revocable by ADDPC and The Arc National at any time without notice for any reason.
**Confidentiality:**
The Participant agrees to maintain the confidentiality of the Program Materials and not to share, distribute, or reproduce them, in whole or in part, in any form or media, without the express written consent of ADDPC and The Arc National.
**Data Collection and Use:**
The Participant acknowledges that ADDPC and The Arc National may collect and process personal data as described in their respective Privacy Policies. The Participant consents to the collection, processing, and use of their personal data as described in the Privacy Policies.
**Termination:**
This agreement may be terminated by ADDPC and The Arc National at their discretion, with or without cause, at any time. Upon termination, the Participant shall cease using the Program Materials and return or destroy any copies thereof in their possession.
**Governing Law:**
This Privacy Agreement shall be governed by and construed in accordance with the laws of the state of Arizona, without regard to its conflict of laws principles.
**Entire Agreement:**
This Privacy Agreement constitutes the entire agreement between the Participant, ADDPC, and The Arc National regarding the Program Materials and supersedes all prior or contemporaneous understandings, agreements, representations, and warranties.
